Please note. If you registered by mail, the first time you vote must be
in-person. You can either vote during absentee voting in-person, or at the polls on election day.
All forms may be mailed, faxed, or hand delivered. Original signature is required.
If you request a mail in ballot, in order for the request to be approved, you must check one of the reasons on the form.
